dwarf_alloc.h, dwarf_alloc.c, dwarf_base_types.h, dwarf_opaque.h: New allocator type (DW_DLA_CHAIN_2), to allow a list of addressed types (address, offset). dwarf_query.c: Use the new allocator type.
