Replace __cplusplus comparisons and dialect __has_feature checks with TEST_STD_VER.
This is a huge cleanup that helps make the libc++ test suite more portable.

Walter Brown sent a list of tests which needed 'additional includes' to match what was in the standard. Added these includes to the tests. No changes to the library or test results.
