7c3769df62 Update more file headers across all of the LLVM projects in the monorepo
to reflect the new license. These used slightly different spellings that
defeated my regular expressions.

We understand that people may be surprised that we're moving the header
entirely to discuss the new license. We checked this carefully with the
Foundation's lawyer and we believe this is the correct approach.

Essentially, all code in the project is now made available by the LLVM
project under our new license, so you will see that the license headers
include that license only. Some of our contributors have contributed
code under our old license, and accordingly, we have retained a copy of
our old license notice in the top-level files in each project and
repository.

6420803549 Fix PR39749 - Headers containing just #error harm __has_include.
This patch changes <experimental/foo> to use #warning instead of
is harmful to common feature detection idioms.

We should also consider only emitting the warning when __DEPRECATED is
defined, like we do in the <ext/foo> headers. Users may want to specify
"-Werror=-W#warnings" while still ignoring the libc++ warnings.

018a3d51a4 [Libc++] Use #pragma push_macro/pop_macro to better handle min/max on Windows
Summary:
This patch improves how libc++ handles min/max macros within the headers. Previously libc++ would undef them and emit a warning.
This patch changes libc++ to use `#pragma push_macro`  to save the macro before undefining it, and `#pragma pop_macro` to restore the macros and the end of the header.

907c1196a7 Add markup for libc++ dylib availability
Libc++ is used as a system library on macOS and iOS (amongst others). In order
for users to be able to compile a binary that is intended to be deployed to an
older version of the platform, clang provides the
availability attribute <https://clang.llvm.org/docs/AttributeReference.html#availability>_
that can be placed on declarations to describe the lifecycle of a symbol in the
library.

0becc206e3 Restore the _NOEXCEPT on the dtor of bad_optional_access. Destructors are noexcept by default, so it's not really needed, but the other exception classes have the _NOEXCEPT, and gcc complains if these are missing. I think we should remove them all - but not today.

e2261bde80 Change the base class of std::bad_optional_access. This is a (subtle) ABI change, and is in response to http://http://wg21.link/LWG2806, which I *expect* to be adopted in Kona. I am making this change now in anticipation, and will get it into 4.0, because (a) 4.0 is the first release with std::optional, and (b) I don't want to make an ABI-change later, when the user base should be significantly larger. Note that I didn't change std::experimental::bad_optional_access, because that's still specified to derive from std::logic_error.

c3589a8305 [NFC] Rename _LIBCPP_TYPE_VIS_ONLY to _LIBCPP_TEMPLATE_VIS
The name _LIBCPP_TYPE_VIS_ONLY is no longer accurate because both
_LIBCPP_TYPE_VIS and _LIBCPP_TYPE_VIS_ONLY expand to
__attribute__((__type_visibility__)) with Clang. The only remaining difference
is that _LIBCPP_TYPE_VIS_ONLY can be applied to templates whereas
_LIBCPP_TYPE_VIS cannot (due to dllimport/dllexport not being allowed on
templates).

This patch renames _LIBCPP_TYPE_VIS_ONLY to _LIBCPP_TEMPLATE_VIS.

b9231a2326 Fix PR30260 - optional<const T> not working.
This patch fixes PR30260 by using a (void*) cast on the placement argument
to placement new to casts away the const. See also http://llvm.org/PR30260.

As a drive by change this patch also changes the header guard for
<experimental/optional> to _LIBCPP_EXPERIMENTAL_OPTIONAL from _LIBCPP_OPTIONAL.

14c09a2413 Add an _LIBCPP_NORETURN inline function named __throw_XXX for each exception type we define. They either construct and throw the exception, or abort() (if exceptions are disabled). Use these functions everywhere instead of assert()ing when exceptions are disabled. WARNING: This is a behavior change - but only with exceptions disabled. 4ad67e08cb Add all the relational operators to std::experimental::optional. Also update bad_optional_access to match the Library Fundamentals draft standard. This is not all of the upcoming changes to optional, though.

b9536101dc NFC. Move definition of _LIBCPP_ASSERT into __debug header and remove external include guards.
Things done in this patch:

1. Make __debug include __config since it uses macros from it.

2. The current method of defining _LIBCPP_ASSERT is prone to redefinitions. Move
the null _LIBCPP_ASSERT definition into the __debug header to prevent this.

3. Remove external <__debug> include gaurds. <__debug> guards almost all of its
contents internally. There is no reason to be doing it externally.

This patch should not change any functionality.
