debugger: IV must not truncate addresses to 16-bit
...
IV was casting to 16-bit, breaking 32-bit code (DPMI) by truncation.
Remove the narrowing cast so IV matches SV/LV behavior.
Tested IV/SV/LV with 32-bit addresses: auto-replacement + save/load OK.
Closes #5851

CPU: The 386 debug registers do not actually do anything yet, so to know when they are used, add a debug message when written. Reduce log spam by fixing XMS driver not to emit "... is enabling flat real mode" unless the CPU is in real mode and flat real mode is now active when it was previously not active.
